The physiological sigh: the fastest way to calm down
For the moment stress spikes — before you speak, after bad news, when the chest tightens.
What it is
Two inhales through the nose — a normal one and a short top-up — followed by one long exhale through the mouth. Your body does this on its own a few times an hour, and always after crying: it is the sigh that comes before you settle.
The second inhale re-inflates the small air sacs (alveoli) that collapse during shallow breathing, so the long exhale that follows offloads more carbon dioxide than an ordinary breath. The long exhale slows the heart. One to three repetitions is usually enough to feel the difference.
What the evidence says
Balban and colleagues at Stanford (2023, Cell Reports Medicine) randomised adults to five minutes a day of cyclic sighing, box breathing, cyclic hyperventilation or mindfulness meditation for a month. All groups improved; cyclic sighing produced the largest improvement in positive mood and the biggest drop in resting respiratory rate. It is one trial and the effects are modest, but it is a well-run one, and the technique costs nothing to try. The article below goes through the design and the numbers.
Where it fits
Use it for acute resets. For the baseline — how you breathe the other 23 hours — slow nasal breathing and a rising Control Pause do the work. The two are complementary: the sigh handles the spike, the daily practice lowers the spikes.
The physiological sigh
- Inhale through the nose, a normal comfortable breath.
- Without exhaling, take a second short sip of air through the nose, to the top.
- Exhale slowly through the mouth, all the way out, longer than both inhales together.
- Repeat once or twice if needed. Three is usually the most you want.
- For daily practice, as in the Stanford protocol: five minutes of repeated cycles, once a day.

- How many physiological sighs should I do?
- One to three for an acute reset. More than that can make you light-headed. The five-minute daily version is a separate practice with its own pace.
- Why the second inhale?
- It reopens collapsed alveoli so the exhale that follows is more effective at removing CO₂ and the lungs work at full surface. It is the difference between a sigh and just a long exhale.
- Is the physiological sigh better than box breathing?
- For dropping arousal quickly, yes. In the Stanford trial daily cyclic sighing also beat box breathing on mood over a month. Box breathing keeps its place for focus and steadiness.
- Can I do it through the nose only?
- Yes. The mouth exhale is slightly more efficient for a fast reset; a nasal exhale is fine and better if you are working on nasal breathing all day.
